CONSUMER
GRIEVANCE PROCEDURE
If a consumer believes that any of his/her rights have been neglected or violated through the provision of service received through DCIL, he/she can go through the following grievance steps:
1. A consumer must start a
grievance procedure within thirty days of the act or decision he/she questions.
2. The consumer should go to
his/her Independent Living counselor who will help to write a statement that
lists the incident or decision being questioned, including who was involved,
the action they want taken and the outcome he/she desires.
3. If the concern cannot be
solved, the consumer can take his/her concern to the Executive Director.
4. The Executive Director will
review, investigate and make recommendations within thirty days of receiving
the written grievance.Ý (This is the
final step of the grievance process.)
Documentation
of the grievance and steps taken, will be kept on file for two years.
The
Dakota Center for Independent Living will not restrain, interfere or
discriminate against any consumer initiating or involved in the grievance
process.Ý The Center staff will not deny
any consumer the right to initiate the grievance process.
Grievance
decisions decided in favor of the consumer will not result in retribution by
the DCIL staff or any of its agents.Ý
Every attempt will be made to resolve a grievance in a timely manner to
the satisfaction of all parties involved.
