Consumer Rights

 

This is to assure that ÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝÝ, a consumer at the Dakota Center for Independent Living has been informed of his/her rights as a consumer of services at the time of the Intake.

 

Consumer Rights Are:

 

1.     The right to choose/or be involved in determining services.

2.     The right to private meetings and to communicate with any persons associated with the Dakota Center for Independent Living.

3.     The right of each consumer, his/her immediate family or legal guardian to present complaints on the behalf of the consumer to the program staff or the director, without fear of reprisal , coercion, discrimination or restraint.

4.     The right for the consumer, his/her family/legal guardian to change/add or withdraw from the program.

5.     The right to be treated courteously, fairly and with the fullest measure of dignity.

6.     The right to be free from mental and physical abuse.

7.     The right not to be discriminated against.Ý All applicants that have been denied services must be notified in writing.

8.     The right to legal representation.

9.     The right to waive the development of an Independent Living Plan.

10. The right of each consumer, his/her immediate family or legal guardian to request a case transfer.
